I can not shift into first gear or reverse from stop. Once the car is in gear and rolling shifts are smooth and no gnashing of teeth. I don't remember when I replaced the master / slave cylinder washers, but I'm sure it has been awhile. It seems there is little in the way of adjustment, at least I have never had to do anything but replace the hydraulic washers and reinstall the master and slave cylinders. Opinions / suggestions?

Most likely the clutch fork taper bolt has sheared which requires the box to come out

To check the pin, remove the slave cylinder return spring and pump the pedal a couple times. If the clutch starts working, then the pin is sheared. If it makes no difference, then you could have air in the system or a bad cylinder.

Sounds like a clutch problem. Does the clutch slip in 4th gear trying to go up a big hill, at like 30mph?

One of my first problems in the British car World was a complete hydraulic failure of the clutch. What struck me was having the car in direct drive and still moving with no clutch and not being able to shift. If you have a problem going into first or reverse, try pumping the pedal a couple of times to see if the clutch works better going into first.

I have also had a broken pin move the pedal up so high the clutch would not work in the lower gears without grinding

Later I had a problem with a different car that would not go into first or reverse without grinding. It turned out to be a bent shaft on the reverse gear idler.

Is the master cylinder losing fluid? Do you see any leakage? If not, bad seals in either are unlikely. Only possibility is leakage back into the reservoir from a bad valve seal (see the shop manual, p. 2-104). Frankly, I also think that a broken pin is likely.
