Two most common in my experience are broken taper pin, and clutch slave assembled incorrectly. The workshop manual only shows the earlier Lockheed slave arrangement; while the vast majority of TR3s on the road have the later Girling slave (and front disc brakes).There are three of us in this discussion that are having the same problem and one other that I have spoken to. If there is a mechanical problem that keeps the slave from being adjusted what could it be? Clutch, pressure plate, throw out bearing? Would you venture an opinion?
Defective new slave cylinders are certainly a possibility, but I've not heard any confirmed reports of that. It's pretty dang simple, not much to go wrong unless it leaks. Wrong parts could be a possibility too, especially if you're working on a car that originally had front drum brakes. I think the Lockheed pushrod is shorter than the Girling pushrod.

FWIW, I believe the Girling clutch slaves were all 1" bore for TR3 through TR4A. Lockheed, IIRC, was even larger, maybe 1.25"
