I had an 85 toyota truck in college. the 3 nuts on the exhaust flange were conatantly working loose. I finally broke down and bought new lock nuts for it and ta-da! it never happened again.
The flange on the down pipe looks like it may be unbolted or down from the manifold. I used to have this problem in the past when the shops would bend the pipe for me before they were available from aftermarket sources.

Sorry to hear that Emma is not well!
When you re install the flange you may consider using different lock nuts such as the expanding, AKA Flexloc or distorted thread lock nut. I have been using these more and more lately and they don't ever seem to loosen up on their own.
