Can't help with the screw, but I have a perfect reproduction handle from our own Editor Reid; I'll start the bidding at $500 ...
Hey, I have a few of those remaining and would take far less. Make me an offer!
Backstory: They were made by the prisoners at the United States Disciplinary Barracks (military prison at Fort Leavenworth, Kansas) in 1991 when I was stationed there (Fort Leavenworth, not the prison). The prisoners were allowed and encouraged to take on various handicrafts and such as a way to train them in skills that might come in handy in their post-confinement life.
I commissioned a run of reproduction jack handles, loaning them an original Shelley jack handle as an example (the simple kind that is basically a simple rod about 15 or 18 inches long - don't remember exactly and I don't have one handy - with the sharp edges at the ends beveled slightly, then cad plated). I was impressed with the final product. I've sold a number of them over the years and still have maybe a half-dozen remaining.
I also had a Jensen Interceptor painted by the prisoners while I was stationed at Fort Leavenworth. You pay only for materials, but that's another story.
