It appears that the shoulder belt has a 2 hole tab. Is this simply bolted to the body or is there a double studded plate like this - page 28

Are the 2 other belt ends bolt on like this catalogue, or snap hooks into eyebolts?

NO !!- if it is actually a 67 there will be only one threaded anchor point on the wheel arch. However, earlier models did have two studs for mounting the seat belt anchors---Keoke

How did the 2 other ends fix to the body?
Along the outboard side of each front seat in the floor are two closed eyelets that allow the belts to anchor.Similarly there are two anchors located on the inboardside of the front seats attavhed to the short fixed tranny tunnel.
