MGB-GT Need help with MGB GT rattle

Anyone know how this washer (top right of pic) could have made it into the area above the top hinge recess in the door pillar of an MGB GT? I got this washer out but have more rattle from same spot. Is there some opening to the top of the pillar that I can't find? Nothing in fender well. Maybe from inside cockpit?
