Leather seat restoration?

I have heard that well worn leather seats can be "healed," and that cracks can be filled and re colored? This is my very dirty drivers seat from my '96 Jaguar XJR, the other seats are near perfect. Any insight and leads to products to do this would be great!

Lots of leather care advice recently posted. Looks like Connally Hide Food, Leatherique, Lexol, Zaino, and several others are used by BCF members. Example:
