Probably done in the Boeing paint shop on Boeing field in Seattle.
They do the design on a computer, have a laser "play" it on the side of the plane (using a correcting program to compensate for the curvature of the plane) and mechanics hand paint inside the laser lines.
Not too complicated, I've watched them paint pictures on Grand Coolie Dam on July 4th using a similar program.

I was in that Boeing paint hangar a couple of years ago - very impressive.
I also had the "airline customer tour" (as opposed to the public tour) of the Everett B747 production line and Airbus Toulouse lines years ago when I was doing aircraft delivery inspections at both manufacturers. All very impressive.
