tub advice needed

okay, so i have been going back a forth on this and figured i'd better ask the forum .. i have only two areas of "known" rust-through on my tub. one is a pinhole on the passenger side door ... not worried there ... the other is a series of pinholes on the passenger side A piller. since i am preparing to take the tub for its final blast and primer, i was wondering should i have that section replaced with good metal or are there other options.

the metal around the area looks good from the outside but i was told if there are holes, then there is rust and it will continue to rust unless removed or treated. i can't really get back into this area effectively enough to remove any inside rust.

i am anxious to get the tub primed but i don't want to have to deal with potential rust-through after priming/paint
 
Generally, If there is rust through then I replace the metal 1.5-2 inches surrounding it.
 
Look at Moss for the skin, long or short, for that A piller.
