Shattered CD

Has anyone see a CD do this? Is there anyway that the drive could have caused a good CD to shattered like this?

It happens when the CD is out of balance, or the spindle of the drive is damaged. At thousands of RPM, it doesn't take much of an imbalance to cause a fracture. This is one reason why you shouldn't use paper labels on CDs - they cause imbalance.
