The section index on the front page is hot-linked to the sections, but the links won't work unless you download the file and open with Adobe Acrobat Reader.

I deliberately skipped the sections on how to order in French, Spanish and so on. Everything else should be there. Please let me know if you find any problems or know of any mistakes or useful additions.
