I'd like to see a cell phone that shuts off or refuses to connect if it detects movement faster than a walking or maybe running person would move. Then maybe the clueless wouldn't have them propped on the steering wheel or held in their laps as they drive.

I once thought that way. How would a phone know if you weren't the passenger though?
 
I'm not thinking just of personal vehicles. At work I regularly ride in a truck with other people. If our schedule or route changes, we need to talk to the boss. This can happen several times a day.
 
A configuration like that would play much havoc on busses and trains also - how do you isolate the driver from everyone else (on a train it would be more possible to block signal for the driver since the locomotive is normally separated from the passenger areas I suppose, but having some mechanism for overriding the block would be needed for emergencies).
 
I have an older vehicle and recently got a bluetooth adapter.
On the newer vehicles there is absolutely no excuse for holding a phone.
